How to publish the form on the checkout & thank you page? Like the purchasing feedback form, or some other business form after checkout. Here is an example.

1. Copy the form code. 

2. Open Settings>Checkout and accounts in your store admin.

3. On the Order status page section. Paste the form code copied before in the Additional scripts input box. 

4. Save the changes.
